Dr. Z writes an order for a clinic patient with febrile neutropenia for vancomycin 800 mg IV every 8 hours (send to peds onc clinic on 1st floor stat). Srcr = within normal limits. Wt = 42 kg Please make any recommendations, if any, and write orders accordingly.

Solution

A patient with febrile neutrophenia should be primarily noted for high fever over stipulated period of time. The MASCC score has to be taken to determine whether it is of high or low risk. Low risk requires only oral bacterial therapy while high risk requires intravenous bacterial therapy Oral therapy is done with quinolones and intravenous therapy requires pseudomonal cephalosporin
