And whenever proper skill set and technical standards are missing then it adds up to the reason of failure.

2) Poor deligation and decision making : If proper deliegation of task is not there eg, if a task require a particular skill be it analytical skill , technical skill etc and its given to a person which doesn\'t match the requirement and appropriate decision making is not in place then these results in failure.
