Case 1.      Presenting problem: Sara, age 24, reports feelings of depression, uselessness, and loneliness. She also reports significant weight loss and difficulty sleeping. She indicates that these feelings began when her sister moved two states away. Although she has had thoughts of suicide, she is not seriously considering that now. She states that she feels best when at work and when telephoned by her sister.

Problem history: Sara indicated that her depression began at age 16 when she was in serious car accident resulting in a series of painful facial surgeries. She indicates that members of her family and friends were very supportive of her throughout her physical and emotional trauma. She reports that her depression lifted when she started this job but gradually crept back over the past few months. She has been involved in therapy twice in the past that she found very helpful. She is hopeful that therapy will result in improving her mood, energy level, and social life. Eventually, she hopes to have friends instead of a therapist provide her with the support she needs.

Solution

The first stage will be the assessment that forms the micro phase in which one on one discussion between the doctor and sara will take place.A number of questions will be asked and the patient will be judged to see if she has a depression or not and to find out what kindd of depression is she having.

The second stage will involve the beginning of therapies(mezzo phase) in which counselling sessions to the patient are given and this takes place for a few months,The patient is examined after every few weeks to see if she is showing progress or not.

The third is final stage(macro) in which the doctor finds the patient is not showing any progress.He given her medications and tells the therapy to continue side by side.Regular sessions and examinations of patient is done during this.
