List the five steps to take after a machine or
equipment has been locked out for repair and is
ready to return to normal operatin condition.
Solution
Step-1: Check the area surrounding the machine. Visually inspect and remove any material or tool lying around the machine. Remove all of them
Step-2 : Check for human presence in around the machines. If they are present, ask them to move to a safer place.
Step-3 : Any de-energizing device or lock-out device(s) on the machine must be removed. If a tag is placed, remove it. Similary remove the de-energizing device or lock-out device(s).
Step-4 : The machine operators as authorized, should proceed to check the machine . Necessary adjustment based on machine operators must be carried out. If needed, the authorized personnel can reposition the machine to fit production need.
Step-5 : Test the machine for function. If the machine functions properly, then the machine can resume normal production.
