a. The IB select input were open?
b. The IB select input were shorted to ground?
c. The enable (G) input were open?

(a) When IB select input is open we will get the Main red and Side yellow LED on.   

(b) When IB select input is grounded Main green, main yello and side red LED will be on

(C) When enable in[put will be open all the LED will be off
