The drug reference manual states the following about the safe dose of midazolam for a child older than 2 months who has status epilepticus: V (Children older than 2 months): 0.15 mg/kg load followed by a continual infusion of 1 mcg/kg min Titrate dose upward q 5 mm until seizure controlled, range: 1-18 mcg/kg/min. A child weighing 18 kg is brought into the ED with continuous seizure activity. The ED physician orders a loading dose of 2.5 mg IV midazolam followed by an IV infusion of midazolam. What is the maximum loading dose for this child? Is the ordered loading dose safe?

  Solution
Maximum loading dose for the child= volume of drug per kg of patient x weight of the patient
= .15 mg/kg x 18
= 2.7 mg of midazolam.
Now here the ordered loading dose of 2.5 mg is safe as it is less than the safe maximum loading (2.7 mg) for the child.
