Using the terms capillary hydrostatic pressure, capillary osmotic pressure, and interstitial fluid explain why you could see pitting edema in the lower extremities in a patient with congestive right heart failure.

Answer- In congestive right side heart failure ,right ventricle looses its pumping function and blood may back up in to other area of the body and produce congestion.right side ventricles pump blood into the lungs due to righ side heart failuer right ventricles has difficulty pumping blood in lungs .BLood backs up in blood capilaries which causes fluid retention in lower extremities which can be seen in the form of pitting edema.

cangestive heart failure may be caouse by diastolic disfunction.the wall of the right ventricle has reduced compliance,that is it is abnormally stiff and therefore has reduced ability to fill adequately at normal diastolic filling pressure . the result is the reduced stroke volume .In this condition the ventriculer compliance is reduced by ventricular contractility is normal.

Due to the low strock volume kidney senses less blood flow from the body it try to compenset by retaining more water and salt in the blood due to this water containt and thus hydrostatic pressure of the blood increases inside the vessels.when blood flows through the body it capllery exchange occur between blood inside the capilary and interstitial fluid outside the capilaries (In tissues).capilary exchange depends on the difference of hydrostatic pressure inside or outside of the blood vessels.

If hydrostatic pressure is more inside the vessles the fluid flow from blood vessels to outside and thus out side tissues fill with the fluids and cause pitting adema.osmotic pressure works against the hydrostatic pressure which is due to the protein containt in the blood .here beacuse blood is diluted thus the protein contain will be less and thus inside the vessels osmotic pressure will be less than outside thus fluid flow outside the vesseles.and couse edema.
