1) Safety of a structure is assured with the strength of the structure. If the structure is able to withstand the design loads,then it is considered safe. Safe structure is one which doesn\'t fail/collapse when loads are applied.Safety of a structure is directly related to strength of the structure.

Serviceability of a structure is associated with the deformation/deflections the structure sees due to service level loads. If the structure undergoes a lot of deformation(even though the structure doesn\'t fail), the occupants feel uncomfortable and unsafe because of those high deformations.Serviceability of a structure is directly related to stiffness of a structure.

Yes,it is possible for a structure to be safe but fail serviceability. If we consider a long span beam, there might be scenario wherein the strength of the beam is adequate to resist the loads. However, it is seeing high deflections.
