I don\'t know what cognitive process model was presented to you in class. However, there it goes my explanation about the accident:

1) Security is first. In order to prevent this accident, there are a couple of actions that should be implemented:

- The the electrical technician should have worn the right equipment, including clothing and insulated boots.

- Before opening the electrical panel, he must have read carefully the equipment service manual (as long as he had not worked with it before). Thus, he would have known how does it works.

- If the conductors are energized the danger is extremely high. If the work has to be done while the conductors are energized, the level of security has to be higher and the man must be really careful with all the equipments and conections.

- If the man wanted to identify a conductor, he could have measured the differential of potential that existed between this conductor and another one (until defining some reference, as a ground). Knowing the potential (referred to ground or another reference) of this conductor, he would have known (or at least would have had an idea) of wich was this conductor (if his desire was identify it). However, there are a lot of experimental techniques to make this procedure, however, tugging energized wires is not an option. Security is first.

2) Safety boots with electrical insulation, multimeter (with the voltmeter and amperimeter options) in order to know whether a conductor is energized or not, and long pants in order to avoid directly contact.

3) First error was not reading the equipment service manual, a big failure.


Second error was related to a processing because of being near exposed to the energized electrical conductors.

Third error is an action error, that occured when he tugged the energized wires.
