Dr. Z writes an order for T obramycin 7 mg/kg/dose IV once daily for a 16 YO cystic fibrosis patient admitted for pulmonary exacerbation. (patient weight = 41.5 kg, SrCr = 0.35 mg/dl) Please make any recommendations (if any) and write orders accordingly.

The concentration of tobramycin to be given is 290.52mg/kg/dose IV once daily
Recommendations for the patient are:
airway clearance therapies are used, plasma concentration of tobramycin is required to be monitored.
Serum creatinine levels need to be monitored. in the case of using any nephrotoxic drugs, renal functioning needs to be checked regularly.
High-calorie diet along with supplements should be provided. BMI needs to be checked regularly.
Appropriate amounts of pancreatic enzymes need to be replaced.
Note: The role of SrCr levels, I suppose to be serum creatinine are not considered in the dosage as per my knowledge. if it is considered, then the dosage changes.
