Dr. Gupta is an endocrinologist and has been working in the field for 20 years. You have been interning with her for three weeks and have seen patients with Addison\'s disease, neonatal diabetes, and Hashimoto\'s disease, among others. It is 8:oo am on Monday and you check Dr. Gupta\'s schedule and see that there are two patients scheduled this morning. Patient #1: Jorge Alvarez Dr. Gupta meets with Jorge and you learn the following information. Jorge is a 45-year old male. He works as an accountant and has three daughters. Jorge has been a smoker for 20 years, but has been attempting to quit. His diet consists of about half home cooked meals and half fast food meals, and his favorite beverage is extra sweet ice tea. He has intentions to exercise, but his desk job keeps him pretty stationary during the day. He has recently been taking the weekends with his daughters. Over the past few months, Jorge has been extremely thirst has experienced some tingling in his legs, and has been urinating frequently. He has also felt extremely tired. Recently, he noticed a sore on his leg that has not been healing quickly and seems irritated. His family doctor ordered some quick labs, and then referred him to Dr. Gupta. Jorge blood work and patient information from his chart are listed below.

Solution

1. patients symptoms: for past few months the patient is feeling

a. Extremely thirsty

b.Tingling in his legs.

c. frequent urination .

d.Exteremly tired

e. Sore on the leg not healing.

The patient has a sedentary habit , with no exercise, food habits include consumption of fast foods, smoker and has extra sweet ice tea . these diet coupled with the symptoms are indicative of type II diabetes .

Review of test results:

BMI: 31.9 higher

Triglycerides :225 higher,Blood pressure and pulse rate on higher.

Blood glucose levels higher. sodium and potassium levels are also bit higher.

The explanation to the patient would be higher glucose levels in the blood are indicative of diabetes and this is coupled with high triglycerides , high B.P and pulse which leads to heart problems.Inching towards higher levels of sodium and potassium may lead to diabetic nephropathy.

3. Additional information that can be gathered is about the patients family history whether the parents were diabetic, there cardiac history .
