Help on d. as I seem to be getting the incorrect answer. I am getting -63 x .04 = -2.52 but the book says it should be -2.12 V. Is the book wrong or am I wrong?

yep... The book is showing wrong answer.. your answer is correct. alredy the code in option a) is showing the correspoinding answer -2.52. it can not be the answer for code in d).
