Dr. Z writes an order for a clinic patient with febrile neutropenia for vancomycin 800 mg IV every 8 hours (send to peds onc clinic on 1^st floor stat). SrCr=within normal limits. Wt= 42 kg -Please make any recommendations, if any, and write orders accordingly.
Solution
· A patient with febrile neutrophenia should be primarily noted for high fever over stipulated period of time.
· The MASCC score has to be taken to determine whether it is of high or low risk.
· Low risk requires only oral bacterial therapy while high risk requires intravenous bacterial therapy
· Oral therapy is done with quinolones and intravenous therapy requires pseudomonal cephalosporin
