1. ATC/KLM captain: The ATC gave the KLM flight a departure procedure but did not give the plane clearance for take off. The pilots might have interpreted the ATC\'s instructions wrong due to the fatigue of pilots.

2. There was not a set protocol for communicating critical information like takeoff clearances between the pilots and the tower unlike today where a controller must directly address the flight and state \"Cleared for takeoff\"

3. Overcrowding of the airport( due to diversions from bomb threat at a nearby airport) forcing the planes to use length of the runway for taxiing.

4. The refueling process took 35 minutes and forced Pan AM grounded as the airport had no space.

5. Sounds on the CVR suggested that during the accident the Spanish control tower crew had been listening to a football match and may have been distracted.

6. Bombing at Gran Canaria: Ofcourse both planes would have been there instead of tenerife.
