Patient Summary: Armando G is a 32-year-old male admitted to the ER. History: Onset of disease: patient transported to the ER when found in his apartment by his friend. During ER assessment, patient was noted to have serum glucose level of610 mg/dL. He is not taking any medication on regular basis as he finds them unnecessary. Vital Signs: Weight: 165lb Height: 5\'11\" BP:78/100 Temp: 99.6 Pulse:100 Resp. rate: 24 Medical History: None Medications at home: None Tobacco use: Smoker 1 ppdx10 years Alcohol use: Daily Family history: father-MI: Mother-ovarian cancer, T2DM Demographics Marital Status: Divorced Years Education: 16 Language: English/Spanish Occupation: Computer Software Engineer Hours of work: 8-7 M F, some weekends Ethnicity: Hispanic Religious Affiliation: Catholic Admitting history/physical: Chief complaint: fr end states that Mr. has not been feeling well at work the previous day. He thought he was fighting off a virus. When he did not show up for work and did not answer his cell phone, his friend went to check on him and found him groggy and almost unconscious at his apartment. He called 911 and the patient was transported to UCSD hospital. General appearance: slim, Hispanic male in obvious distress.

Solution

1. I feel he is suffering from type 2 diabetes mellitus since his blood glucose level is too high and his mother also a diabetes mellitus pateint and this type of diabetes is heretable in nature.

2. When the blood glucose is high to the normal level, it means that the cells are not taking the glucose to use as a energy source. This is due to decresed insulin sensitivity of the cell receptors which are resposible for the glucose to enter inside the cells. this condition often reffered as insulin resistance. because of cells lacking glucose and energy that is why the pateint got fainted.

3.the medication metformin is typically recommended but the dietary changes and regular physical exersice are more important than medication so that the blood glucose level should not increased in the pateint. The food should be with low glycemic index to avoid hyperglycemia.  His diet plan should includes very limited carbohydrate intake (30 grams per day).A low-fat vegan diet improves hyperglycemia.

4. Morning breakfast- Scramble 1 egg prepared with olive oil; top with ¼ c chopped tomato, onion, and chile salsa. Serve with toasted 100% whole grain brown bread, spread with 2 Tbsp low-fat (1%) cottage cheese, and 1 cup fat-free milk.

before lunch snack- one low sugar friut . for example-guava, pineapple (100g) or pear with cucumber salad

lunch-  salad with dark greens (spinach) and carrot. 1 bowl brown rice, 2 whole grain bread, any fish curry or vegetable curry and low fat yoghurt

evening snack- 5 almonds with strawberries or died plum or fig
