Solution

There was a problem of solid rocket booster in the design flow which was not addressed by the engineers. NASA had focus on its ambitious schedule and the program managers did not focus on dealing with the situation and they were scared to delay the launch. They were advised to take the role of management. However the engineers did not appropriately report the potential risks and did not follow up with the engineering recommendations.

Yes I do agree it was one of the factors for the disaster. It was straight up an example of poor management decision. The structure did not award the employees who placed importance to safety but instead appreciated employees who believed in pleasure the customers.

If I would have been in the management then I would have definitely placed importance on ethical decision making. There would be importance give to safety and ethical management style.
