The common IT failure that may occur witin the health care systems is that the hospital management systems which keeps records about the patient ,billing details etc is not working.This system may fail frequently due to under maintainence of the software by the staff,even operating the system by unexperienced staff.To minimize the risk of failures ,they may use proper antivirus softwares with continious updates,use genuine softwares so that proper security patches can be obtained at right time,disable pendrive options to control the insertion of pendrives as per their will etc.key roles of management is to give permission for the project to progress.staffs roles is to use the system feectively .stakeholders resonsibility is to give the requirements and funds needed for the successful implementaton of the project.
