The practice manager must find out the ways and means (1 either) to get the finances in order with co-operation from all those who are connected and or interested in the implementation of the comprehensive diabetic care management (2 or ) get the optimal use of the available resources and try to get other necessary (deficit) resources directly in the form of products and services free of cost from sponsors and or donors.

The first step in the decision making process is to gather the facts and figures, may be through brain storming.
Next step is to generate all possible feasible solutions and carry out pre-feasibility analysis. In the present case quantify the minimum possible requirements for implementation of the comprehensive diabetic care management.
Among the alternatives/options available compare their possibility/suitability (evaluation) with respect to implementation of the program and make the best choice.
As per my understanding and based on the personal experience rather than asking for funds it is better to augment the resources directly in terms of products and services from the sponsors and allow them to advertise in return. Otherwise look for the donors and convince them about the relevancy for undertaking such noble cause for social welfare.
