Case Study
Mr. Davis, a construction worker, self-administers ibuprofen 800 mg four times a day for pain related to arthritis and his work. He presents to the physician’s office with stomach pain that has become chronic within the past 2 weeks. The physician orders misoprostol (Cytotec).
1) Why did the physician order misoprostol (Cytotec)? What is misoprostol? How will this drug help with the stomach pain? (3 pts)
2) Mr. Davis asks if misoprostol (Cytotec) is safe for his wife. She has chronic back pain and also self-administers NSAIDs prescribed by her physician. Would you refer Mrs. Davis to her physician for the same treatment? (2 pts)
Solution
1. ANS:
Misoprostol (Cytotec) is a water-soluble, viscous liquid. Misoprostol oral tablets are available either in 100 mcg or 200 mcg of misoprostol.
Misoprostol tablets contains Inactive ingredients like hydrogenated castor oil, hypromellose, microcrystalline cellulose, and sodium starch glycolate.
Action mechanism: Misoprostol can reduced the stomach acid concentration and replaces protective substances in the stomach.
This medication can helps to protects Mr. Davis stomach lining by lowering the amount of acid that comes in contact with it.
2. ANS:
In the most cases of pain reducing treatments physicians prescribed these NSAIDs and aspirin drugs. Misoprostol (Cytotec) is used to prevent the formation of ulcers in the stomach during treatment with aspirin or an NSAID such as ibuprofen (Motrin, Advil, Nuprin, others). So prescribing same treatment can give positive result to Mr. Davis wife.
