Case study: David is a 7-year-old requiring a routine physical examination at the nurse practitioner\'s office. David\'s past medical and family history is not unusual. His activity level is normal, and his mother verbalizes that he is quite healthy. David\'s physical examination is normal until the cardiovascular system is evaluated. He is noted to have a systolic murmur. There are also bounding pulses in the arms and severely decreased pulses in the legs. A pediatric cardiology con­sultation is requested. Which cardiac defect causes such a striking discrepancy in blood flow to the upper and lower extremities?

Solution

Valcular aortic stenosis is a congenital hypertrophic cardiomyopathy mainly involved behind David\'s striking discrepancy in blood flow to the upper & lower extremities. During this stenosis of heart valves (mitral, aortic, tricuspid) radius of the valve is going to decrease (aortic valvular stenosis) result in increased resistance to blood flow from the left atrium to left ventricle finally due to bicuspid aortic valve disease. It is the disease predominantly due to malformation and improper functioning of the blood flow through the bicuspid valves result in abnormal heart sound called as murmur finally a systolic murmur with decreased stroke volume. In these conditions, it is leading to a striking discrepancy in blood flow to the upper and lower extremities & there are also bounding pulses in the arms and severely decreased pulses in the legs due to left ventricular failure. In extremely serious conditions, cardiac arrest with congestive heart failure may attack to the patient due to aortic stenosis.
