Katie is admitted to recovery at 11:20 PM. After 1 hour, her uterus is slightly boggy, lochia is moderate, and her bladder is distended.
11. What are the priority interventions at this time? What is the highest priority nursing diagnosis for the recovering client?
Solution
The vital signs of Katie should be checked forst whcih Iincludes temperature, heart rate, respiration, blood pressure, and pain level.
Katie\'s breats shoudld be checked for redness, bleeding, engorgness, etc. Second the fundus should be checked (the upper part of the uterus). If the fundus is boggy, a massage is needed to the uterus in a rotating motion supporting the lower uterine part until it gets firm. if not, the uterus could invert, and this could result in severe hemorrhage.
