Solution

As our line is perpendicular to given line, y=1/2x + 5

So slope of our line is, m=-1/(1/2) =-2

And it passes through (6,-4),

So, point slope form is

y-(-4) = -2(x-6)

Simplify it further, we get

y+4= -2x+12

y= -2x+8

Which is slope intercept form.
