Solution

Febirl Neutropenic fever is a single oral temperature of 38.3º C or a temperature of greater than 38.0º C ( 100.4º F) which sustained for more than 1 hour in a patient with neutropenia.

The vancomycin is order depends upon weight also so if total amount of Vancomycin is to be given 800 mg for a 42 kg person

it should be indicated as below

Indications for the empiric addition of vancomycin (mg/kg IV q8h) to drug regimens listed above: that is in this case 20 mg/kg/8 h

Continue therapy for at least 7 days until cultures are negative and clinical recovery is noted.
