It has been proposed that estradiol (an ovarian hormone) exerts a negative feedback action on LH secretion via two mechanisms: 1) inhibiting GnRH release from the hypothalamus and 2) decreasing responsiveness of gonadotrophs to GnRH. Briefly describe an experimental approach for demonstrating that estradiol decreases the response to GnRH in women. Assume a group of 20 women who have had hysterectomies (removal of the uterus and ovaries) is available for the experiment.

Solution

The experiment includes 20 women who were studied following bilateral ovariectomy and hysterectomy performed by laparotomy under general anaesthesia.The ovaries were normal and indications for operation were not harmful in effect.The women were divided into two group based on treated with estradiol or not.In group1(n=10), no hormonal treatment to post operatively women. In group2(n=10), women received estradiol through skin patches.The first patch was applied after operation and further on post operative 3rd and 6 th day.

Result and conclusions :- In group 1 women no contraindications for administration of these steroids were identified.Operation was done in early to mid-literal phase of cycle i.e. 5 days after endogenous LH peak detected by measurement in daily blood samples taken from follicle size.In all women pituitary response to GnRH was investigated on daily basis till discharge date from date of operation.This presents pituitary sensitivity to GnRH.FSH and LH were measured in all blood samples.During operation corpus luteum was confirmed.
