2. Share practice improvements utilized from your own clinical nursing experiences that have led to enhanced patient outcomes.
Solution
Post hospitalization services provided by the hospital is an important aspect of health care industry. Hospitals usually think that serving patient in the hospital serves their duty but the real game begins when they leave hospital after the treatment.
The medicare provided by the hospitals offers the third party payment and so they are most effected by these readmisions
1. As quality improvement suggestion for the hospitals will be to provide a full body checkup at the end of the tratment to the patient which will help them analyzing the situation of the readmission of the patient, Moreover they can also provide them sessions on how to manage their disease so that the patient doesn\'t eat unhealthy food or practice the conditions which were trigring their illness before
2. Patient usually come back to the hospital not because they were not treated well but because they again develop the situation within themselves, The reason behind this is that they are unaware about the basic life style causes of the disease, A suugestion is they should be kept informed of the reasons which made them ill. This will pshycologically effect the patient and will help them to prevent themselves again from getting the same disease
