the blood bank received a call requesting 2 units of \"kell-negative\" red blood cells for a patient with anti-k. why is the request incorrect?

a patient has a history of the following alloantibodies: anti-s, anti-leb, and anti-jka. which of these antibodies are clinically significant?

A patient has a phenotype of Le(ab+). Is this patient a secretor or a nonsecretor? what genes are responsible for conferring Lea and Leb antigens on the red cells?

Solution

The request is incorrect because Anti-k is a immune red cell antibody after ABO and Rh blood group system. It is Ig-G class alloanitibody. Patients without specific Kell antigen will develop antibodies against Kell antigen when blood with the antigen is transfused. Due to this hemolysis takes place if the Kell-negative patient is transfused with blood containing anti-k.

Alloantibody anti-s is part of MNS antibodies causes transfusion reaction. Its causes severe to fatal reaction in case of hemolytic disease of newborn. Care should be taken while transfusion.

Alloantibody anti-leb are Ig-G class. This anti-leb does not have any clinical significance.

Alloantibody anti-jka is the part of Kidd antigen system or Jk antigen. they are very dangerous and can cause severe acute hemolytic transfusion reaction. It cannot be detected during pre-transfusion test so, after the transfusion the hemolysis of the blood takes place. It also causes acute extravascular hemolysis lead to delayed hemolytic transfusion reaction, caused death of fetus and newborn.

The patient with Le(a-b+) phenotype are secretors. they are two genes responsible for the conferring the Lea and Leb antigen, Le gene and Se Gene. In case of Le gene codes for fucosyl transferase that adds fucose to the precursor to convert into Lea antigen. In case of Se gene codes for fucosyl transferase that adds fucose to the precursor to produce Lea antigen. If the second fucose is add to precursor ti produce Leb antigen.
