sqrt(x+4)= 4+6x

squaring both sides

x+4=(4+6x)2

x+4=16 + 36x2+ 48x

36x2+47x+12=0

We cant solve it by factoring,so we have to use quadratic formula here

x=(-47+-sqrt(481))/72

And if we check by plugging these values of x,we find that the solution is
