Post means after, Translation means protien synthesis, Modification means changes. hence PTM mean the changes done on a protien after it leaves the ribosome.

The changes could be like

1. Adding an location targetting signal peptide so that protien known where it has to go in the cell.

2. Adding acetate, phosphate, amide, or methyl chemical groups to aminoacids.

3.Cleaving peptide bonds like removing initiator methionine residue.

4. Attachment of lipid to amino acids (lipidation)

5. Other modifications like glycation, carbamylation carbonylation etc
